CSNW Animal Report Form

This form allows you to submit information about crimes, possible crimes, and suspicious or other activity THAT OCCURS IN THE COLONY SURF COMMUNITY. Personal information that is required on this form is held in the highest confidentiality and is only disclosed to the Colony Surf Neighborhood Watch Coordinator and, in case more information is needed, the agency the information is reported to (ex. Mason County Sheriff's Office, Mason County Animal Control, PUD).
Colony Surf Neighborhood Watch would like to remind you that it is YOUR responsibility to contact the Mason County Sheriff's Office by calling 9-1-1 for emergencies or (360) 426-4441 for non-emergencies. The "Tipster" form is NOT a substitute for contacting the authorities. Remaining anonymous is a concern of residents when reporting crimes. You CAN AND WILL be anonymous under MOST circumstances. When calling either phone number to report crimes or possible crimes let the dispatcher know you want to be anonymous and the information is NOT sent to the responding deputy.

How To Use This Form

Do NOT report barking dogs on this form. Barking dogs falls under the Colony Surf Covenants and should be reported to the Colony Surf office by calling (360) 877 5434.
Make sure to answer all questions when applicable. More information rather than not enough is asked for so that the incident you are reporting can be properly addressed. Information you provide is anonymous and confidential.
Please report any suspicious or criminal activity to 9-1-1 or (360) 426-4441 (this can be done before or after you submit the information to Colony Surf Neighborhood Watch). By doing this you can be assured that the reported activity is dispatched to an ON-DUTY deputy.
Required questions must be completed before you are able to submit the form to the Colony Surf Neighborhood Watch. You will see a confirmation on your screen letting you know that the form was sent.

For wild animals let us know if the animal was in your yard, attacking another animal or person, foraging in the garbage, etc.. For strays state "stray animal". If you are reporting an abused/neglected animal provide information about living conditions, starvation, injuries, etc..For dangerous dogs let us know about aggressive characteristics, any bites inflicted on another animal or human, etc..
